Dr. Perkins, a trauma counselor, comes to help the hospital for recovery and also to evaluate if doctors are ready for work. Owen & Cristina plan their marriage. Derek returns to work, but makes Meredith worried by continuously speeding while driving.

Meredith tells Derek about her miscarriage and is permitted to work. Cristina starts experiencing PTSD, and Alex is denied to work by Bailey as he didn’t get a bullet from his chest removed. Derek’s sister visits him but things go awry between them, Chief helps Alex with his trauma of elevators, and Derek and Meredith help Cristina in her post-traumatic stress. Derek and Meredith receive a disturbing news from Obstetrician, and Lexie becomes angry after seeing Meredith and April getting closer. Attending surgeons fight for a million-dollar grant, while Derek tells Chief that he wants to begin a clinical trial for Alzheimer’s as he fears Meredith will develop it. A film crew arrives at the hospital after six months of shooting to make a documentary about the recovery of patients and doctors, while Mary Portman returns for an operation by Bailey after which she does not wake up which devastates Bailey.

Arizona receives the elite medical grant so she has to go to Africa for which she and Callie decide to go together, but later she refuses to take Callie which breaks their relation. Cristina keeps a housewarming party in which April bursts into tears after an encounter with Alex leading Jackson to beat him. Dr. Robert Stark, Pediatrics, joins the hospital. Cristina spends the time of her own party on the terrace by talking and having drinks with Derek. The residents do night-shift, and they are left shocked by seeing Cristina working as a waitress at Joe’s when they go there to celebrate Derek’s clinical trial grant. Derek takes Cristina for a fishing trip to help her, and Lexie asks Mark’s help when a nurse refuses to follow her orders. Arizona returns, but Callie refuses to meet her. Every resident has to teach a medical student, and they receive negative evaluations by students except Alex as his student becomes one of his flings.

Teddy has to make a life-changing decision for her legal spouse who is also her patient. Arizona wants to return to Callie who refuses telling that she is pregnant with Mark’s baby. Derek starts his clinical trial of Alzheimer along with Meredith. Bailey tweets during surgeries, which is firstly opposed by Webber but later he agrees seeing it as a valuable learning tool. Mark tells Lexie about Callie’s pregnancy for which first she decides to give him second chance but later gets into a relationship with Jackson, and Callie has to decide roles of Arizona and Mark in her baby’s life. Thatcher come back with kidney stones along with his 20 years old girlfriend which shocks Lexie, and Alex fights with new OB/GYN but later wins her respect. Webber offers Meredith to continue her mother’s clinical trial on diabetes, but she chooses Alzheimer trial. Meredith tries to prove that she can be Chief resident, Bailey sneaks off with Eli and does a mischief, and everyone is surprised to see Adele as one of the ER patients.

Meredith has to choose between her eyesight and her fertility treatment. Mark proves that he is taking his role of father-to-be very seriously to Arizona and Callie. Richard wants Meredith and Derek to accept Adele into their trial desperately. Mark and Arizona argue about the baby shower, and Teddy freaks out when Henry gets one more health complication. Residents’ competition for Chief’s position increases when Richard gets green signal for his diabetes clinical trial. The season shows the first musical episode when Arizona and Callie have a car accident. Everyone helps Callie and baby Sofia during their recovery. When Arizona and Callie’s wedding arrives, they realize that it is not like they planned.

All are jealous of Alex as he seems as a top contender for Chief’s position, Perkins gives Teddy a tempting proposition, and Derek and Meredith makes a life-changing decision. Owen conducts formal interviews for Chief’s position. Cristina becomes more defiant, and Lucy & Alex’s relationship gets tested. Meanwhile, Jackson suddenly backs out from Webber’s clinical trial. Meredith’s dishonesty in clinical trials for Adele is revealed by Alex which leads to surprising consequences for all. Cristina has to choose between her relationship and career as she is pregnant, finally deciding to abort the baby. Owen announces his decision about Chief Resident, and Teddy makes a shocking choice related to her love life and future. Derek leaves Meredith with Zola, the baby that they are trying to adopt, and Alex is kicked out of Meredith’s house.
