The Malayalam television series Sithara is a remake of the popular Hindi TV drama Sasural Simar Ka. Sasural Simar Ka was a highly successful show that aired for many years in Hindi on Indian television. Sasural Simar Ka aired for almost seven years on Colors TV channel, from 2011 to 2018. With its long broadcasting run, Sasural Simar Ka emerged as the second longest-running show in the history of Colors TV, after another family drama series, Balika Vadhu, which also featured actress Avika Gor. The show centers on two sisters, Simar and Roli, living in Vrindavan.

Simar dreams of being a dancer one day. Her marriage gets arranged with a wealthy businessman named Prem Bharadwaj. However, on their wedding day, Simar participates in a dance competition while Roli ends up marrying Prem instead, creating complications. Prem's brother Siddhant marries Roli subsequently. Over time, both Simar and Roli get accepted into the Bharadwaj household. Another woman named Khushi marries into the family as well, seeking vengeance for her sister's untimely death. She becomes a surrogate for Simar and Prem's child Anjali to later blackmail Simar for ownership of the Bharadwaj property on behalf of her lover Veeru. Though the property papers get retrieved, the fallout leaves Roli presumed dead when Veeru shoots her. Fortunately, Simar finds Roli alive later on and brings her home safely. In due course, Simar and Roli find their long-lost sister-in-law Jhanvi, who has now killed her husband Shaurya. More relatives come into the picture, like politician Jwala Devi, who briefly kidnaps Roli at one point. Roli also unintentionally kills a woman named Sunaina, whose husband Vikrant then asks Simar to pose as the deceased Sunaina temporarily.

Two years go by with Simar living as Sunaina and raising Vikrant's daughter Sanjana while Roli takes care of little Anjali. Eventually, Simar returns to the Bharadwaj family fold. Vikrant gets jailed for trying to kill Prem, Sanjana becomes Simar and Prem adopts a child. From here, the story progresses with the family confronting threats from supernatural creatures with mystical powers. These include shape-shifting snake-woman Maya, evil dayans (witches) like Mohini and Indravati, demons, ghosts, and more. Saving the family requires excellent sacrifice at times - Roli tragically loses her life protecting Simar from one threat. A mourning Siddhant later remarries Prerna and relocates abroad. In one plot twist, the demon Kaal impregnates Simar with an evil child possessing spooky abilities. Simar gives birth to son Piyush, but the family doubts the baby's intent. Heartbroken, Simar leaves with Piyush before eventually returning home. Twenty-one years later, grown up, Piyush and Anjali reunite with their family members.

Much drama follows - marriages, divorces, vendettas, violence. Ultimately Simar kills antagonist Bhairavi who had killed Simar once before. Goddess Durga also makes an appearance to revive Simar and destroy evil. Ultimately, Piyush finds love with Avni as the Bharadwaj family comes together again. Following the popularity and high viewership of Sasural Simar Ka, the show was remade into various other Indian languages to target regional audiences. One of those remakes is the Malayalam series Sithara, which has taken the original Hindi drama and adapted it for Malayalam-speaking channels and audiences in the state of Kerala.
